Welcome to L4 Module 5: Community care  

The module aims to provide the learners with the ability to apply the strategies to support a person with an acute/chronic health and wellbeing condition, recognise and respond to changes and support, mentor and facilitate a person to maximise independence in health and wellbeing setting. 

Weekly tasks (Quizzes, forum discussion, article & video discussions) can be found in each module. Learners will follow clear instructions to submit completed work.
