ACP.505 Application of Health and Welling Models

Aim:

The Aim of Module 5 is to explore the use of Healthcare Models for the development of health and well-being outcomes and associated strategies. The course then explores the application of and enhancement of those strategies, based on findings and the particular demographics of clients and family/whānau.

 

Learning Outcomes:

  • Compare Māori and Pasifika healthcare models in order to integrate with applied dementia care planning practices.
  • Develop Te Ao Māori planning approach using Māori healthcare models to negotiate and identify client and family/whānau health and well-being needs and aspirations.
  • Select and apply strategies related to assessments and diagnosis, awareness and risk reduction to inform, educate and support family/whānau caregivers.
  • Critically select and apply a range of dementia care practices inspired by Te Whare Tapa Whā healthcare model in order to support identified health and well-being aspirations of whānau.
  • Apply advanced dementia and aged care strategies in dementia care practice in order to respond with sensitivity to diverse whānau contexts.
