Welcome to Module 3: ACP.503 Dementia Care Planning

Aim:

The Aim of Module 3 is to explore the development of dementia care plans and associated dementia care strategies. The course introduces reflective and reflexive practices for improving both care provision and planning practices.

Learning Outcomes:

  • Analyse needs and aspiration dimensions integrating the client’s demographic factors in-order to inform dementia care plans. 
  • Negotiate and develop a personalised, coordinated and enabling, person-centred care plan integrating identified client health and wellbeing aspirations.
  • Select and justify strategies related to day-to-day living, maximising wellbeing, and health in order to enhance a person-centred dementia care plan.
  • Develop, enhance and apply strategies in a dementia care plan integrating client and whānau needs and aspirations in an aged care environment.
  • Critically reflect on and evaluate care support practices using a Te Whare Tapa Whā framework in order to improve health and wellbeing outcomes.
  • Evaluate personal aged and dementia care planning practice in order to enhance professional competence in the dementia care sector.
