Aim:
The Aim of Module 2 is to explore the application of Healthcare Models to Healthcare Practice and supports the development of collaborative care strategies and subsequent delivery.
Learning Outcomes:
- Select, Develop and Apply Dementia Care Strategies which promote collaborative, Whakawhānaungatanga and Kaiwhakatere-based approaches to support for Dementia clients.
- Provide collaborative aged and Dementia care support practice using applicable Dementia Care Services while considering the wider political and economic factors which impact the health and wellbeing sector in New Zealand.
- Select, justify and apply collaborative relationship-building approaches using Te Wheke informed practices to support identified health and wellbeing aspirations of whānau.
- Apply considered and sensitive collaborative care strategies which respond to the client’s individual diverse context in order to maximising their standard of living, and health and wellbeing outcomes.
